mailform.php is a simple one-page html form mailer that's gathered quite a few features over the years.

• Automatically learns the host name of the server its installed on - you just add the local user name. This is a spam control measure, since the form can only be used to send mail to addresses on the domain it's installed on.
• Optionally logs all submissions
• Handles file uploads
• Redirects users to a page of your choice - no graphic design tweaks needed
• Specify required fields, fields to include (or not to) in the email, message subject, and From: address
